This document sets out the terms and instructions for providing counseling and/or psychotherapy ("Services") to service users ("Users")
It is your responsibility to understand and comply with these Terms and Conditions.
1. We understand that life will inevitably have its ups and downs. Through counselling, a counsellor will work with you to explore the problem and its crux, clarify your feelings and thoughts, and try to come up with a corresponding action plan.
2. The counseling process is a two-way relationship. The user's honest expression of his or her situation and cooperation with the counsellor's action plan are the key to the effectiveness of the counselling, and it is also the user's responsibility.
3. A complete counselling phase is very important for effectiveness, so it is recommended that you complete at least the service phase (6-8 times). You can decide after discussing with the psychological counsellor.
4. Users are not allowed to record, record, take photos or take screenshots during counselling.
5. Change or suspension: If you want to change the counselling schedule or suspend counselling, please be sure to discuss your ideas with the psychological counsellor in the previous counselling to ensure the best interests of the user in changing or suspending the counselling.
6. Counselling psychologists, counsellors and social workers do not provide legal-related services (such as testifying in court), nor do they provide written reports (such as psychological evaluation reports, professional certification reports, diagnostic certificates).
7. I will not use Counselling services if I am experiencing an immediate crisis or have thoughts of suicide, homicide, or harm to myself or others. If I encounter an emergency crisis, I can call 999, go to the nearest emergency hospital, or call the community crisis hotline (Samaritan Suicide Prevention Hotline 27908844; Suicide Prevention Hotline 23820000) for immediate help.
1. An appointment is required for each interview, and Users are required to attend on time. Session time will not be made up for latecomers.
2. Each session of personal psychological counselling lasts 60-90 minutes depending on the reservation, and charges are based on each session. If Users require more than the reserved time, please make a longer appointment reservation, with confirmation and payment before the session.
3. Any changes to the time and date of the session requires 48 hours notice. If notice is less than 48 hours, the fee will be deducted for that session.
4. If Users are late for 15 minutes or are absent, the fee for that session will be deducted.
5. If Users want to change the time (with 48 hours notice) please notify Soulgood.
6. For time changes, late arrivals or absences, charges will be subject to Soulgood.
7. If there is a typhoon signal No. 8 or above or a black rainstorm signal, the session for the day will be canceled and Users need to contact the Soulgood system to make an appointment for another time.
8. Our therapists will not provide legal-related services (including testifying in court).
9. Personal information provided by Users will only be used for the counselling services of this center. Counsellors will collect and store your personal information in accordance with the privacy regulations and processing policies.
Confidentiality Agreement:
1. Counsellors will follow professional counselling ethics; the content of the session, the personal information of the User, interview records, will be kept confidential for the purpose of providing counselling services and will not be transferred to a third party. In order to provide proper and quality counselling services, users' progress will be monitored and consulted anonymously. Exceptions will be made to confidentiality unless the User has the following circumstances:
a. When you are in danger of harming your own life or safety or the life or safety of others, or are in danger of committing suicide and need to notify relevant personnel;
b. When required by relevant legal provisions;
c. When you are a minor and your family members request access;
d. Disclose relevant information to others with my consent or authorization;
e. Emergency situations (such as emergency medical assistance).
2. All information is used in the best interests of the user, including referrals to other professionals with the user's consent.
3. Personal information provided by service users will only be used for the counseling services of this center. Relevant personal data will be stored in Soulgood (Breez Health Limited) system. Counsellors will also collect and store your personal information in accordance with the Privacy Ordinance and Processing Policy.
I hereby declare that I have read the entire Soulgood Counselling Notice and Consent Form and sign it to express my agreement and understanding of the above terms.
